Announcement of Dr. Dawodu's arrival at Albany Medical Center, Albany, NY Dr. Dawodu closed the medical practice portion of this business and moved back to academics and is currently an Associate Professor of Physical Medicine & Rehabilitation/Interventional Pain Management at Albany Medical College/Albany Medical Center (one of the oldest private Medical schools in the US established in 1839 with the hospital established in 1849) with added practicing privileges at Capital Region Orthopaedics both in Albany, NY. Dr. Dawodu still provides Independent Medical Evaluation (IME) and chart review along with Medico-legal advice, Healthcare business consultation/solutions, risk management/ethics review, utility/resources review, etc at spare time and can be reached through the above numbers or email address .
Patients requesting their old notes can contact Dr. Dawodu and fax to him a RELEASE OF INFORMATION CONSENT FORM (this is MANDATORY under HIPAA rules and regulations and there is a charge for collating, copying and mailing the notes).:-
